State-required Vaccinations Needed For Students (Grades K, 7, 12)
Parents and Guardians,
Please make certain your child has had the following vaccinations and doses prior to the start of the 2019/2020 school year.
If your child is going into kindergarten they will need the following vaccinations / total doses during their lifetime:
Four or More Doses of DTap/DT (or any combination)
Three or More Doses each of Polio and Hepatitis B vaccines
Two Doses of each: MMR & Chicken Pox vaccines
If your child is going into 7th grade:
As in previous years, incoming 7th graders will need to receive a Tdap (Tetanus, diphtheria, and pertussis) booster and one dose of meningococcal vaccine prior to the start of school.
If your child is going into 12th grade:
Two doses of Meningococcal Vaccine are required prior to the start of 12th grade. Only one dose is required if given on or after the 12th grader’s 16th birthday.
